Transitions of care are for post-hospital stays. Chronic care management is for disease management. Principal care and wellness are for disease process intervention.

How should I prepare for my appointment?

This is done for you. You will receive communication prior and a link to your phone on the day of the meeting.

What do I bring to my Appointment?

Please bring any questions. Otherwise, everything is handled, “come as you are”.

How do I submit insurance?

You will be walked through this while making an appointment.

Who is my provider?

Local, highly certified NPs, PAs, DOs and MDs. Transition appointments are with APPs and all management is overseen by physician supervision.
